Print an Order Form to Mail In

We have eliminated our old PDF order form which had a table of shipping rates based on the dollar total of your order, but you can still print an order form to mail in. Here's how.

1.

Please use the shopping cart to collect the items you want to order.

Click the "Printable order form" button on the left side of the shopping cart page, then enter the shipping destination (with zip code for the US). Sales tax applies to orders shipped to Texas, so be sure to check the "Texas" checkbox if appropriate. Incorrect information will result in wrong shipping charges and may delay your order.

3.

Click the "Print order form" button. Your order form will display with your items and the exact shipping for your location.

4.

Print it, fill in the address and other information, and mail it to:
